Fall from Height Leads to MOL Fine

A Goderich-based window and door manufacturer has been fined $65,000 for a violation of the Occupational Health & Safety Act (the Act). The incident took place in September, 2008 and resulted in a worker falling and suffering wrist and head injuries.

According to the MOL, the worker was dismantling the roof of a sound enclosure. The worker unhooked the fall protection line and moved towards a ladder to get down. The worker stepped on an unsecured plywood plank. The plank slipped and the worker fell four metres to the ground.

Dashwood Industries Limited pleaded guilty under the Act to failing to ensure the plywood was 'secured in place' as required in by section 15(a) of the Regulation for Industrial Establishments (Reg. 851).

15. A cover on an opening in a floor, roof or other surface shall be, 

(a) secured in place;
